The current knowledge of data on the frequency of psychiatric diagnosis and treatment in emergency medicine is diverse. The purpose of this study was to extend the knowledge of pre-clinical data especially in an urban region like the Ruhr-gebiet, Germany. We analyzed all of the emergency protocols (DIVI 98 Version N-4.0+n-4) of the City Essen in 2007. A total of 14,526 records (85.9%) could be analyzed. The frequency of psychiatric diagnosis was close to 6.5 %. Intoxication, attempted suicide and a state of arousal were the most common psychiatric emergencies. The result of this study is not very different to others. But there seems to be a local exception. The group of patients with severe intoxication was hospitalized mainly at hospitals with internal medical care units and not at purely psychiatric hospitals. The use of psychopharmacotherapy in an emergency pre-hospital setting is still very rare.
