Eliminating tobacco use in mental health facilities: patients' rights, public health, and policy issues.

In 2006, a report from the National Association of State Mental Health Program Directors showed that individuals with serious mental illness die, on average, at least 25 years earlier than the general population.6 Cardiovascular or heart disease is the leading cause of death among patients with serious mental illnesses, resulting in more deaths than from injuries or obesity-related diseases such as diabetes.7 A survey of 222 state-operated psychiatric facilities in the United States found that the majority of hospitals had begun the process of becoming tobacco-free facilities or were planning to do so in the subsequent year.3 Facilities that had undergone these changes indicated improved patient health, cleaner indoor environments and hospital grounds, increases in staff satisfaction, and more time to provide treatment. These findings are consistent with national trends calling for a transformation of US mental health care to be more oriented toward wellness and recovery.
