What is the best available evidence regarding the promotion and support of self-care management for adults living in the community with physical chronic illness during the patient-practitioner encounter? Joanna Briggs Institute mixed method systematic review with meta-synthesis. Chronic illness rates continue to rise in the Western world, giving rise to the importance of looking at new models of care that move from viewing patients as passive recipients to developing partnerships between chronically ill patients managing their own care, with support from healthcare professionals. Nurses, doctors, dieticians, podiatrists and community healthcare workers can all play a part in this collaboration, partnering with patients with chronic illness to assist them in actively managing their own care.
