We describe the successful extraction of a foreign body from the peripheral airway using flexible bronchoscopy during surgery. A 76-year-old man was admitted to our hospital with a nail-shaped shadow on his chest radiograph. He had aspirated a nail 3 months earlier but did not seek medical assistance at the time. A chest radiograph and computed tomography examination revealed a nail in the peripheral S7 region of the right lower lobe, The nail could not be extracted with forceps during flexible bronchoscopy, and video-assisted thoracic surgery was performed. During surgery, the nail was not detected even with an additional minithoracotomy, and was consequently found in the truncus intermedius using fluoroscopy. The patient was reintubated with a single-lumen tube and the nail was extracted using flexible bronchoscopy. Peripheral airway foreign bodies can be difficult to detect in patients with few complaints. We recommend that during surgical extraction of a radiopaque foreign body from the peripheral airway, surgeons be prepared to use intraoperative fluoroscopy in case the foreign body migrates as a result of surgical manipulation.
