A 77-year-old male developed hemodynamic instability seven days following a mitral valve replacement with a #27 mm St Jude mechanical prosthesis (St. Jude Medical, St. Paul, MN, USA). Transthoracic echocar-diography showed only a 10-mm pericardial effusion, but multidetector computed tomography (MDCT) identified a posterior free wall rupture of the left ventricle (Fig. 1). The patient underwent emergent repair of the rupture using pledgeted mattress sutures and fibrin glue. He tolerated the procedure well and has no evidence of a pseudoaneurysm six months following surgery.
