A 23-year-old female was admitted with palpitations.A transthoracic echo revealed a secundum atrial septaldefect (ASD) (Fig. 1), and a left ventricular myxomatousmass in the lower septum (Fig. 2). At the timeof surgery, the left ventricular mass could not be visualizedthrough the secundum ASD, so a 2-cm incisionwas made parallel to the left anterior descending(LAD) toward the apex, which revealed a 2 × 1.5 ×1.5 cm myxomatous lesion in the lower septum attachedvia a small stalk (Fig. 3). The mass was excisedwith a small rim of septal tissue, the ventriculotomyrepaired with two dacron patches using a sandwichtechnique, and the secundum ASD closed with a run- ning 4-0 prolene suture. Pathology confirmed that themass was a myxoma. The patient had an uncomplicatedpostop course and a repeat postop echo showedno residual ASD or mass lesion.
