The introduction of hypothermia by Bigelow at the University of Toronto1 represents a milestone in the history of cardiac surgery. It is ironie that, years later, a group of cardiac surgeons in the same university would question the need for hypothermia. In an evolution of thinking, based on many years of dedicated basic science research, these surgeons would develop new concepts of myocardial protection aimed at keeping the heart (and the patient} normothermic. Warm Heart Surgery was a major paradigm shift from accepted methods and techniques of myocardial protection. Hypothermia had dominated the field of cardiac surgery and played such a major role in the history and the development of specialty. It is important to recognize that, at the time, all equipment and materials used in heart surgery were designed to withstand hypother-mic conditions. These included not only oxygenators and tubings, but also cardioplegia delivery systems, aimed at cooling the blood from 37degC to 15degC in one passage of blood through the circuit. The belief that "the colder the better" was applied to many conditions, without any demonstrable beneficial effects.
