Targeted radiation for melanoma: alpha therapy prepares for a beta test.

Patients with locally advanced or metastatic melanoma have few therapeutic options that provide long term survival benefits. The incidence of melanoma has been steadily rising in the United States with estimates of nearly 60,000 cases of melanoma for 2005 with almost 8,000 deaths attributable to this malignancy. The combination of increased patient numbers and a deficiency in effective treatment options has been the impetus for both intensified basic science research and an increase in clinical investigations. Currently clinical trials evaluating the efficacy of chemotherapy, biologic and vaccine based immunotherapy, and radiation therapy are ongoing.Melanoma cells were once thought to be relatively resistant to radiation therapy, however, the routine use of external beam radiation in the palliative setting has demonstrated that local control for metastatic melanoma can be achieved. Although somewhat controversial, these observations have been interpreted to reflect unique radiobiologic characteristicsof melanoma that predict increased efficacy with higher dose per fraction regimens. In the case of ocular melanoma, a more precise role for radiation therapy in local tumor control has been delineated with the use of both ~(125)I plaque brachytherapy and proton therapy. Another pre-clinical radiotherapeutic modality that has recently been developed is the delivery of alpha particle emitting radioisotopes. This strategy of radioimmuotherapyhas the advantage of greater relative biologic effectiveness through the delivery of short range, high linear energy transfer particles (reviewed in ref. 4), a characteristic that may be particularly advantageous for the radiobiology of melanoma.
