A 62-year-old man presented with severe headache, nausea and vomiting, dysarthria and ataxia. He had transient lateral deviationof his left eye and left facial droop in the ambulance. On examination, he was dysarthric with mild right arm ataxia. A lumbar puncture was negative for xanthochromia and his non-contrast CT brain was unremarkable. The CT angiography of the posterior circulation is shown in Fig. 1.
