Keratoacanthomas with perineural invasion (PNI) are rare, with an incidence of 0.2% of all examples of the entity.Venous invasion is even rarer, with only a handful of cases reported.2'3 To have one of each in the same sign-out of 240 cases, each from a different patient, seems worthy of documentation. The case with PNI represented the 129th case personally seen by the author in the last 20 years and the case with venous invasion was the 11th (several were seen as consultations). None of the patients has suffered any untoward complications, but less than one third of these cases has been subject to formal follow up.4 One case that caused great concern was from the lip of a middle-aged woman with both perineural and intraneural invasion of numerous large nerves up to 0.8 mm in diameter. An e-mail message to the clinician confirmed a disease-free survival of 18 months so far.
