Increased levels of human neutrophil alpha-defensins in chronic venous leg ulcers

Chronic venous leg ulcers (CVLU) are characterized by bacterial colonisation, ongoing and unregulated inflammation and excessive proteolysis, chemotacticactivity and endothelial leakage. Initial pathogeneticsteps involve activation of endothelial cells and extravasation of inflammatory cells into the wound area. The high influx of leukocytes and subsequent release of proteases from polymorphonuclearneutrophils (PMN) leads to high proteolysis in the surroundings of chronic ulcers. These findings, together with the observation that keratinocytes display a non-migratory phenotype in CVLU leads to a failure of re-epithelialization (for review see ). PMNs contain various antimicrobial peptides and proteins involved in innate defence against invading microbes. One significant group, the defensinslocated in azurophil granules, are secreted into the phagolysosome following phagocytosis of bacteria. However, upon stimulation with potent stimuli, defensins may also be released extracellularly from PMNs . Recent findings indicate that defensins may exert functions beyond antimicrobial activity, being involved in chemotaxis and activation of antigens-presenting cells, thus belonging to the group of "alarmins" involved in various aspects of antimicrobial defence .
