A 22 year-old man was found wandering in a field, having sustained an assault with multiple stab wounds to the neck and severe swelling to his face. Upon initial arrival at a non-trauma center, he was cooperative and moving all extremities purposefully. However, because of an uncommon dialect, the health care providers were unable to communicate with him or assess his orientation. He had significant swelling of the left side of his face, with his eye swollen closed and left hemotympanum. His multiple deep stab wounds to the anterior aspect of the neck included a 1-cm midline laceration below the jaw and a 3-cm laceration just to the left of the jaw at the level of the cricoid cartilage. Air was bubbling from this second laceration. A third superficial laceration was present just above the right clavicle. The lacerations were not actively bleeding. His initial vital signs were as follows: blood pressure, 130/92 mmHg; heart rate, 132 beats per minute; respirations, 26 breaths per minute; and oxygen saturation, 100% on high-flow oxygen. Two large-bore intravenous lines were established, and he was intubated for airway protection. A chest radiograph revealed appropriate tube placement and subcutaneous emphysema in the neck. He remained tachycardic and tachypneic with a stable blood pressure and was transferred via helicopter to the trauma center.
