Introduction: Since the identification of sickle cell disease (SCD) more than 100 years ago, studies have determined that affected patients are at increased risk of certain infections. The natural course of SCD frequently leads to functional asplenia, chronic ischemia, and a dysfunctional immune system, which predisposes patients to infection by encapsulated organisms, specifically Streptococcus pneumoniae? Before the initiation of universal newborn screening for SCD and the subsequent use of penicillin prophylaxis as well as the introduction of vaccination against S pneumoniae, a child with SCD had more than a 300 times greater relative risk of bacterial meningitis than the general population. Since the initiation of penicillin prophylaxis and vaccination with pneumococcal polysaccharide vaccine (PPV-23) and pneumococcal protein-conjugate vaccine (PCV7), the incidence of all Spneumoniae infections in patients with SCD has declined by 68% to 90%. Despite these significant improvements in childhood mortality in persons with SCD, the age of death of an average adult patient with SCD is between 42 and 48 years, approximately 30 years less than the average life expectancy in the United States.8 Furthermore, adults with SCD continue to die as a result of multiorgan failure of unknown etiology as well as acute chest syndrome, in which infectious organisms may be involved.
