Hand surgery usually involves a combination of local or regional anesthesia and varying degrees of sedation. Sedation is a broad term. Anxiolysis, or minimal sedation, may be as simple as giving a patient 1 oral dose of a benzodiazepine medication and should allow patients to respond normally to verbal commands. Conscious sedation, or moderate sedation/analgesia, commonly refers to the administration of intravenous medications to control pain and allay anxiety. At this level, the patient should be able to respond purposefully to commands and light tactile stimulation. With minimal or conscious sedation, cardiovascular function should be maintained, and the patient should be able to breathe normally and protect his or her own airway. In deep sedation, repeated or painful stimulation is required for response and the patient's ability to maintain his or her own airway may be compromised, putting the patient at risk for cardiovascular and respiratory complications. There is no clear line between levels of sedation; it is easy to inadvertently render a patient deeply sedated.
