In their article, Loyo-Varela et al. present a clinical overview of the treatment of pituitary adenomas, offering a retrospective analysis on a large patient series (3004 patients) most of them operated transsphenoidally over a 38-year period. It clearly represents a praiseworthy contribution to the main pertinent literature in terms of surgical experience, mainly with the use of the transsphenoidal approach to the pituitary, using the microscope as a visualizing tool in most of the cases. This technique can be considered a milestone of the last 50 years, since its introduction in the 1960s by Jules Hardy. Credit should be given to the group for having collected such a huge amount of information, which is added to other reports of similar weight around the world, first of all the contribution of Edward Laws with more than 5000 transsphenoidal surgeries, but all of them are considered points of reference for surgeons, scientists, and patients. In the last century, the microsurgical transsphenoidal approach has yielded excellent global results in the surgical treatment of pituitary lesions, with minor limits of the procedure itself and very good compliance by the patients, as compared to the transcranial option for the same condition.
