A 42-year-old woman, gravida 3 para 3, presented for a tension-free transobturator tape procedure for mixed urinary incontinence that had not improved despite tolterodine therapy. An intraoperative cystoscopy with intravenous indigo carmine was performed after insertion of the transobturator devices (Fig. 1). At this time it was noted that the patient had 2 left-sided ureteral orifices and 1 right-sided ureteral orifice, each with separate peristalsis and urine jets. The transobturator tape procedure was completed as planned.
