Living with chronic illness is now the norm with our rapidly growing, ageing population. The dominant treatment for chronic illnesses is prescribed medications. New medications are being developed for people with chronic conditions to take for a lifetime while waiting and hoping for a cure. It is generally accepted that the effectiveness of medications and their long-term outcomes depend on the person's adherence to their prescribed regimen on a regular basis (Sabate 2003). However, if a person is prescribed multiple medications, certain issues require careful consideration. Medicine adherence of >80% is an accepted cut-off point for defining a therapeutic level of medicine adherence (Haynes et al. 2008). Yet, about 50% of people do not take their medications as prescribed with non-adherence increasing in the presence of chronic conditions (Haynes et al. 2008). Medication mismanagement and safety are a cause of great concern and cost to healthcare providers worldwide (Miller et al. 2007).
