Torsion of the uterus is denned as rotation by more than 45° of the uterine body around the long axis of the uterus. It is an extremely rare condition with most reported cases seen in a gravid uterus. The presentation is variable and nonspecific, making diagnosis difficult. We report on a case of uterine torsion and the associated diagnostic difficulty, the first such recorded in our institution at the University Hospital of the West Indies.A 50-year-old nulliparous female, with a 10-year history of uterine fibroids, presented with a 3-day history of abdominal pain, vomiting and loose watery stool. Before presentation, she had seen a private practitioner and been given analgesia, antacid and proton pump inhibitor. The abdominal pain worsened and by the time of presentation was generalised, non-radiating and not associated with fever or diarrhoea. She had a history of recurrent urinary tract infection but denied any current urinary symptoms.She reported regular menstrual cycles with a duration of 6 days. There was no history of menorrhagia or dysmenorrhoea. She gave a history of cone biopsy performed 10 years earlier. Subsequent Pap smears were normal.
