Dear Sirs,Acute vertical gaze palsy is one of the main clinical manifestations of rostral midbrain and paramedian tha-lamic infarctions, in the vascular territory of the posterior thalamo-subthalamic paramedian artery. This artery lies between the bifurcation of the basilar artery and the origin of the posterior communicating artery [1]. Diffusion-weighted MRI (DWI-MRI) is a sensitive method for the detection of rostral midbrain strokes [2] and contributes to improve clinico-anatomical correlations in vertical ocular motor disorders. We report a patient with a left unilateral rostral midbrain infarction on DWI-MRI who developed conjugate downward and upward gaze palsy.
