I sometimes fantasize that the diseases I treat have human personae. Psoriasis is the in-your-face used-car salesman who just will not give you a moment's peace. Mycosis fungoides is the passive-aggressive middle manager trying to take over his boss's position; he wants to be seen just enough to have a presence but not so much as to make corporate waves until it is time to pounce, opportunistically, on his target. And then there's syphilis. Ah, syphilis! Syphilis is the Cold-War-era international spy. It generally tries to stay below the clinical horizon. Yes, it occasionally surfaces as a rash or by producing nonspecific illness, but even then it tries not to call attention to itself by being asymptomatic and often subtle. It may try to avoid detection by masquerading as other dermatoses. Its modus operandi is to damage its host subtly, gradually, but relentlessly, all the time remaining undetected. We physicians often find ourselves seeking it out, dragging it from the shadows into the light of diagnostic clarity.
