(A) Acute pupil-sparing right third nerve palsy in a 7-year-old child. The patient complained of headache and had a similar episode 1 year previously. The pupils were brisk with normal response to light and accommodation similar to the fellow eye. (B) Photograph 12 days after a course of oral steroids shows near total recovery.
