Endoscopic biliary stent insertion is a well-established treatment for hepatic, biliary and pancreatic disorders [1]. Exceptionally, the biliary endoprosthesis can be accidentally inserted in the portal vein. Fistula formation between the biliary tract and the portal vein is rare [2], with potentially fatal consequences. Here we report the case of a liver transplant recipient who had this rare complication.
