Tests for anti-neutrophil cytoplasmic antibodies (ANCA) are commonly performed to evaluate patients suspected of having small vessel vasculitis [1-3]. We previously published a paper in this journal detailing an evidence-based algorithm we had developed to optimize ANCA testing . The algorithm uses ELISA assays for PR3- and MPO-ANCA followed by confirmatory testing of all ELISA-positive sera by an indirect immunofluorescence (IIF) assay on human neutrophil substrates. This sequence provides a very high degree of sensitivity and specificity for detecting ANCA-associated diseases (e.g., Wegener's granulomatosis and microscopic polyangiitis).
