摘要

A 34 year old woman was referred for evaluation of possible Sneddon's syndrome. She has longstanding livedo reticularis and Raynauds syndrome. Over the past 6 months, she developed increasing fatigue and her family felt she had become inattentive and uninterested in life. She frequently forgot what she was doing and has accidentally left entire meals on the stove to burn. Five months ago, an extensive neurologic evaluation was completely negative including an MRI and examination of cerebrospinal fluid. She has been treated for depression for the past 4 months with no improvement. An episode where she was found unresponsive prompted a repeat MRI which demonstrated multiple small white matter lesions, diffuse atrophy, and an acute infarct. Her past medical history is remarkable only for two miscarriages approximately 2 years ago. Her family history is negative with the exception of her mother having a similar skin condition.
