In their review article, the authors describe the mechanism of action of emergency contraceptives. They state that the main action of ulipristal acetate (UPA) "is to delay ovulation without any effect on the endometrium" and "does not prevent fertilization or implantation." However, in the same article, they say that there is an UPA effect on the endometrium but dose dependent in higher doses while lower doses equivalent to the 30 mg used for emergency contraception (EC) were similar to placebo — citing Stratton et al. [1]. According to that publication, however, there is a significant effect on the endometrium in all doses. A similar effect on the endometrium could be demonstrated when 50-mg UPA was given in the early luteal phase [2]. The 50-mg UPA capsule used is pharmacokinetically identical to the 30-mg micronisation formulation in the EC randomized trials [3]. These endometrial effects suggest mechanisms by which UPA might be effective as an emergency contraceptive and may hamper implantation [2].
