To the Editor We congratulate Pinto et al on another excellent piece of work. Once again they show us that image guided prostate biopsy using multiparametric magnetic resonance imaging (MRI) improves detection efficiency compared to standard transrectal biopsy, as defined by the number of cores required to diagnose 1 man with prostate cancer.The authors might consider using a more clinically meaningful end point, such as the proportion of men with clinically significant cancer, rather than their chosen end point, which was detection of any cancer. We anticipate that targeting for disease that is clinically relevant would result in superior detection overall, in addition to detection efficiency, compared to 12-core transrectal ultrasound guided biopsy. Moreover, it is probable that the men diagnosed as a result of targeting would be less likely to be diagnosed with indolent cancer
