The new resuscitation guidelines were awaited at the end of 2005 with tension and excitement - it was almost as emotionally charged as the publication of the latest Harry Potter novel. In the first few hours after the guidelines were released, the European Resuscitation Council's servers were completely overloaded and crashed several times as a result. This is not a phenomenon that has been noted with any other medical guidelines - plenty of which are currently available!The example illustrates very clearly the dilemma in which emergency medicine is currently caught up. It is extremely difficult to carry out scientifically high-quality investigations in emergency medicine in humans - in the context of prospective, randomized studies, for example - and such studies are therefore extremely rare. This is why only a few first-class publications are sufficient to initiate a radical paradigm shift . Parts of existing strategies - and sometimes even the whole of them -are called into question in the process.The 2005 slogan 'back to the basics', or rather the philosophy behind it, goes to the heart of the matter here - and not only in the field of resuscitation. Rigorous simplification is the first step needed to implement emergency medicine expertise on a broad front. This is all the more important for laypeople, as emergency medicine always means working in conditions of extreme stress. The educational strategies used to communicate information in the field of lay resuscitation, therefore, have to be adapted even more rigorously to what is practicably feasible. As I see it, more people could be saved if almost everyone were to carry out pure cardiac compression . Recent data are available on this topic, and these data are already being debated. It may perhaps be time to teach resuscitation even more simply - and to teach it to people at an early age, in every single school. I would like to thank Jerry P.
