Cardiovascular disease (CVD) is the leading cause of morbidity and mortality in Western countries. Numerous randomized trials have shown that therapy especially with statin drugs have reduced cardiovascular events and, in some instances, mortality. Sachdeva et al. [1] analyze the Get With The Guidelines database for coronary artery disease (CAD) hospitalizations from 2000 to 2006 with documented lipid levels in the first 24 h of admission. There were 231 986 hospitalizations from 541 hospitals with admission lipid levels obtained within 24 h of admission during the period of 2000-2006. Admission lipids were documented in 59.0% of the patients with mean LDL levels of 2.7mmol/l (105mg/dl) [2]. It is remarkable that 49.6% of the patients hospitalized with CAD had LDL levels less than 2.6 mmol/l (100mg/dl) and 17.6% of them had LDL less than 1.8 mmol/l (70 mg/dl). Among patients with prior history of CAD, other atherosclerotic vascular disease or diabetes, 56.4% had LDL less than 2.6mmol/l and only 29.4% were receiving lipid-lowering medication before admission. On the basis of accumulated evidence from epidemiolo-gical studies and randomized clinical trials, the ATP III guidelines have identified LDL as the primary target for cholesterol-lowering therapy and the LDL goal for patients with CAD, other atherosclerotic vascular disease or diabetes is less than 2.6mmol/l [2].
