Emerging role of cellular cholesterol in the pathogenesis of nonalcoholic fatty liver disease

Nonalcoholic fatty liver disease (NAFLD) is a highly prevalent disorder with the hallmark being excess accumulation of hepatic triglycerides. It encompasses a wide spectrum of liver abnormalities, ranging from simple steatosis or nonalcoholic fatty liver (NAFL) to, with chronic inflammation, nonalcoholic steatohepatitis (NASH); the latter being a strong risk factor for progression to fibrosis and cirrhosis. NAFLD is also linked to other nonhepatic manifestations including the metabolic syndrome and cardiovascular diseases. The pathogenesis of NASH is multifactorial. Apart from the well established dysregulation of triglyceride and fatty acid metabolism, other pathogenic factors, including oxidative stress and endoplasmic reticulum stress have also been implicated [1]. In recent years, increasing attention has been given to the potential pathogenic role of hepatic free cholesterol accumulation.
