Strain of the internal oblique muscle, also known as side strain, is a rare injury and is characterized by the sudden onset of pain and focal tenderness over the rib cage. This unusual injury has occurred during various sports, including baseball, cricket, javelin throwing, golf, and tennis (1-5). The typical mechanisms of injury are trunk rotation and eccentric activity of the internal oblique muscle, which occur during movements, such as throwing, swinging, or twisting movements, and usually occur on the side of the nondominant arm (1,2,6). Herein, we describe a case of internal oblique muscle strain on the side of the dominant arm in a representative Japanese handball player. To the best of our knowledge, no cases of internal oblique muscle strain on the side of the dominant arm in a handball player have ever been reported. This unique motion is common in handball and theoretically increases one's risk for internal oblique muscle strain on both the side of the nondominant and the dominant arm.
