摘要

Survival among patients on hemodialysis (HD) with diabetes mellitus is worse compared to nondiabetic patients. This study is aimed to get insight of the characteristics of patients on hemodialysis in UKI Hospital Jakarta 2011–2015, and compare the survival of hemodialysis patientswith diabetes mellitus as comorbid and the of the patients without diabetes mellitus. This study used secondary data that collected from UKI Hospital Jakarta’s medical records of patients diagnosed with chronic kidney disease (CKD) and end-stage renal disease (ESRD). Number of subjectsin this study is 105 patients. Study design was retrospective cohort with four years duration from January 2011 to December 2015. The variables included are age at first HD, gender, education, insurance type, vascular access, cardiovascular, hypertension, and anemia. The data analysis wasperformed using Kaplan-Meier and Cox Proportional Hazard. Based on the data of 105 patients, 38 patients (36.2 percent) were dead during the study, 45 patients (42.85%) were still on HD until the end of the study, 3 patients (2.85%) were hospitalized and 19 patients (18%) others disappearfrom observation. All the patients who did not die were 67 people (63.8%) and were referred as censored cases. The number of HD patients with comorbidity diabetes mellitus (DM) 41 patients (39.05 percent) and 64 patients (60.95 percent) were non-DM patients. The characteristics of most ofthe patients who undergoing HD at UKI Hospital included: does not have diabetes, age at first HD was less than 53 years, male gender, education level above high school and using national Insurance (ASKES), vascular access using cimino, had a hypertension, did not have cardiovascular, and neverhad anemia. Diabetic patients on HD had 3.8 times the risk of mortality compared to non-diabetic patients. Furthermore, patients with education lower than high school had 4.6 times the risk of dying compared to patients with higher education. Sex has no effect on the survival time of patientson HD. There are differences in survival between diabetic and non-diabetic patients in hemodialysis. Survival of non-diabetic patients is better than patients with diabetes as a co-morbid. This study obtains the differences of the variables that affect the survival between diabetic and non-diabeticpatients. The significant confounding variables that effect survival on hemodialysis patients by comorbid diabetes and non-diabetes is education, source of cost hemodialysis, vascular access, and hypertension.
