A plethora of surgical approaches and techniques for orbitotomy have been published since its first description in 1841 by Parker.1 The earliest described techniques of orbitotomy included anterior and lateral transorbital approaches mostly aimed at removing tumors confined within the orbit. The transcranial approach, as a method to tackle tumors involving both the orbit and anterior cranial vault, was first introduced by Dandy in 1922.2 Dandy's original technique involved removal of the frontotemporal bone and exposure the orbital roof with preservation of the orbital rim. Over the years, this procedure has undergone several modifications, including removal of the orbital rim for better exposure and accessibility.3'4 In this review, the authors discuss the various transcranial approaches to the orbit with special emphasis on surgical details, indications, advantages, disadvantages and more recent modifications.
