Mycoplasma pneumoniae infections are among the most common bacterial infections that occur in children; they can sometimes be severe, spread systemically, and evoke autoimmune reactions affecting multiple organ systems. Despite the potential for significant illness in some children, a microbiological confirmation is often not sought because of the perception among some physicians that there are no satisfactory diagnostic tests suitable for the acute care setting, that treatment with macrolide antibiotics is always curative, and that the majority of infections, even if untreated, will eventually resolve without sequelae. While these beliefs that are held by many primary care physicians and reinforced in medical education programs to some extent, there is much that has been learned about mycoplasmal respiratory infections in the past several years that warrants reconsidering these concepts.
