The regional pharmacovigilance centre in Rennes, France, reported a case of hypersexual behaviour in a 71 year old male patient with Alzheimer's disease, taking donepezil, a cholinesterase inhibitor. After a few days of treatment, his wife complained of unusually frequent and inappropriate sexual advances. This behaviour disappeared after stopping donepezil (I). In the French pharmacovigilance database, two cases of hypersexuality attributed to donepezil were identified, in female patients aged 85 and 88 years, after taking 10mg of donepezil per day for 1 month and 3 months respectively. In both cases, the problems regressed after stopping the drug and reappeared after restarting it (1). Five other detailed reports of hypersexuality attributed to donepezil have been published (1). In four cases, the disorders arose 4 to 8 days after the start of treatment with donepezil at a dose of 5mg per day, and in the other case a fortnight after the dose had been increased from 5mg to 10mg per day. The patients were taking other drugs, but it was the discontinuation of donepezil which was followed by regression of the problems in a few days. In two cases, reintroduction of donepezil was followed by the reappearance of the problems (1).
