Prevention of rickets-400 iU of vitamin D per day for light-skinned children

In a randomised trial in about 1000 light-skinned children, conducted in Finland, daily supplementation with 1200 units (IU) of vitamin D3 until the age of 2 years did not increase bone density or reduce the incidence of infections compared with daily supplementation with 400 units. In children, rickets results from vitamin D deficiency which can be due to low exposure to sunlight. Its main effects are seizures, bone deformities and an increased risk of respiratory infections (1). Vitamin D supplementation is justified for the prevention of rickets in children under the age of 1 year living in France, or in western European countries at the same latitude. Vitamin D overdose can cause severe and potentially fatal hypercalcaemia (1). Depending on the degree of skin pigmentation, evaluation data have shown that daily supplementation with 400 to 600 units (IU) of vitamin D2 (ergo-calciferol) or vitamin D3 (colecalciferol) appears sufficient to prevent rickets. These doses are not associated with an increased risk of hypercalcaemia, provided any vitamin D intake through formula milk is taken into account (1).
