Noscapine is one of the opioids used to treat cough. Overall, in this situation, opioids rarely have a favourable harm-benefit balance.Their efficacy is modest and they carry a risk of the adverse effects and drug interactions associated with opioids. In addition, noscapine carries a risk of angioedema (1,2). Over the course of 2017, the Dutch pharmaco-vigilance centre (Lareb) and the World Health Organization pharmacovigilance centre (Uppsala) collected and analysed reports of abdominal pain (89 cases), upper abdominal pain (28 cases), chest pain (64 cases) or chest discomfort (9 cases), linked to noscapine.The patients were aged 8 to 76 years, when the age was known. The symptoms usually began within hours following noscapine administration, often between 30 minutes and 4 hours. In 41 patients, abdominal pain, including upper abdominal pain, resolved after noscapine was discontinued. The symptoms reappeared in 6 patients after restarting noscapine. In 74 cases, noscapine was the only drug used. Several patients described the pain as: a " terrible pain, I was quite sure I had a heart attack or an aneurysm"; "feels like I can hardly breathe" (3,4).
