In adult patients suffering from acute, nonspecific low back pain without radicular pain, what data are available from the comparative evaluation of drugs used for pain relief? We carried out a review of the available evidence, following the standard Prescrire methodology. In one double-blind, randomised trial in 372 patients, the efficacy of oral diclofenac or oral ibuprofen was superior to that of placebo, but was only modest. The efficacy of naproxen is probably similar, but it has not been evaluated in this clinical setting. Based on the adverse effect profiles, it seems preferable to choose ibuprofen, or possibly naproxen. Nonsteroidal anti-inflammatory drugs should be avoided in women throughout pregnancy, and in those who might be pregnant.
