In mid-2017, the French drug regulatory agency (ANSM) again reported errors in prescription, dispensing, preparation or administration with the three different injectable formulations of amphotericin B available in France. There is a so-called conventional formulation of amphotericin B, the deoxycholate (Fungi-zone0), and two lipid formulations: a complex phoaspholipid (Abelcet0) and a liposomal form (AmBisome0) (1,2). The dosages of these three formulations are different. Most of the errors involved administration of Fungizone0 in place of the liposome product (AmBisome0), using the dose and rate of administration of AmBisome0. The consequences were an overdose of amphotericin B, creating a risk of serious cardiac and renal damage (1). Amphotericin B is an anti-fungal with a narrow therapeutic index, i.e. there is only a small margin between therapeutic and toxic doses (2).
