As soon as possible, establish a relationship with the agitated patient that is soothing and reassuring, by adopting a calm and caring attitude and isolating the patient from anything that appears to be contributing to the agitation or that could pose a danger. When psychotropic medication appears justified, an oral benzodiazepine such as diazepam or lorazepam is the first choice and often suffices when the patient accepts it. When oral administration is not appropriate, the alternative is intramuscular injection of a benzodiazepine such as lorazepam (or diazepam or dipotassium cloraze-pate if lorazepam is not available) or a neurolep-tic such as haloperidol. Physical restraints are sometimes necessary.
