摘要

Most endoscopic procedures in rural areas are performed by general surgeons. This provides a unique opportunity relating to the diagnosis and management of gastroesophageal reflux disease (GERD). It gives the rural surgeon an unfiltered exposure to the entire spectrum of GERD. Two recent developments are shaping the future of antireflux surgery. First, many reports have demonstrated side effects of prolonged use of proton pump inhibitors, such as osteoporosis, dementia, kidney disease, etc. Second, there are numerous reports of the safety and durability of endolumenal options for GERD such as transoral incisionless fundoplication (TIF) and nonablative radiofrequency to lower esophageal sphincter (Stretta).
