Background: Insulin use in the inpatient setting often occurs in a non-standardized way including sliding scale use. At our institution, this practice was compounded with the existence of multiple non-standardized insulin order sets with unclear and unhelpful names in the hospital's order entry system, ultimately leading to low use of the insulin order sets. Description: In order to simplify and better align inpatient insulin ordering with the Canadian Diabetes Association practice guidelines, we developed 5 standardized inpatient insulin order sets for commonly occurring clinical situations using Plan-Do-Study-Act (PDSA) cycles.
