Non-Hodgkin lymphoma (NHL) primarily involving the central airways is uncommon. Most primary endobronchial lymphomas involve the main or the lobar bronchi. The trachea is an extremely uncommon site of involvement by NHL and primary lesion being more unusual than the secondary. We describe a case of NHL primarily arising at the carinal region and clinically mimicking a carcinoid. A 35-year-old woman receiving treatment for bronchial asthma was referred to the chest clinic for evaluation of continued breathlessness for the past year despite treatment. She was most uncomfortable lying on the left lateral position. This patient had a history of admission in medical emergency 1 month ago for severe breathlessness and wheezing. She was then intubated and ventilated for a day. She had normal chest radiograph
