In their response (pp. 2543-2544) to our recent study on the cost and feasibility to implement the global strategy for the reduction of mother-to-child transmission (MTCT), Dr Sera Young and colleagues argue for the inclusion of infant feeding counselling and the promotion of exclusive breastfeeding as part of the intervention package. Although we agree with their comments, we did not include infant feeding counselling as one of the interventions to reduce MTCT because of lack of documented data. Indeed, to perform a cost analysis of infant feeding counselling intervention, we would need data such as the prevalence of exclusive breastfeeding, how long women breastfeed, how much time counsellors spend with their clients each session, how often clients visit the clinic or how often counsellors carry out home visits for adherence counselling; to our knowledge, there are no studies fully documenting such data, thus we did not have information available on which we could base our assumptions.
