摘要

Histoplasma is a dimorphic fungus that presents; as an opportunistic infection frequently reported in HIV-infected patients who reside in endemic areas. In the Americas, Histoplasma capsulatum is the predominant form and it is most frequently found in North and Central America. The most common clinical presentation is pulmonary histoplasmosis; however, in immunosuppressed patients, other organs can be affected with no lung involvement. A 44-year-old woman, who was HIV-positive, presented to our ward with a 3-week history of bilateral hand and foot pain. The patient had been on antiretroviral therapy (ART) for her HIV for the past 7 years. However, she stopped the treatment 2 months before the presentation to our hospital. Her CD4+ cell count at the time of presentation was 12 cells/(xl and her HIV viral load was 121 785 copies/ml. Initial bilateral hand and foot radiographs showed no abnormalities.
