Dr. Reich attempts to discount our rinding of increased "Sarcoid-like" granulomatous disease in World Trade Center responders, a pattern of disease that peaked 2-3 years after the attacks of September 11,2001 and then resolved. He argues that our observation reflects a combination of ascertainment bias and accrual of previously unrecognized cases [Reich, 2011]. We disagree with his conclusions for the following six reasons: 1. Our findings are biologically plausible. It is well established that the 9/11 responders faced a complex and unprecedented exposure to airborne dust and debris. Especially those responders who were caught in the dust cloud that rolled through the streets of lower Manhattan on September 11, 2001 inhaled massive quantities of particulate matter, large and small, into their airways and their lungs. Airborne concentrations were so high that they overwhelmed all normal physiological defense mechanisms. Bronchoscopies performed on September 11 responders have documented the presence of large particles deep in their lungs.
