Pisani warns against some possible unwanted consequences of the efforts to achieve universal health coverage (Essay, 21 September). In particular, an unconditional push forthe lowest possible prices of essential drugs might lead manufacturers to cut quality assurance costs, increasingthe risk of poor quality drugs or of manufacturers' withdrawal from the market. Focusing on minimising the direct cost of drugs fails to considertheir value—a cheaper, substandard drug might harm patients, and failure to cure will eventually impose higher costs on the health system. Quality and affordability can go together—as shown by the World Health Organization's prequalification programme and the Medicines Patent Pool.
