It is encouraging to see HIV discussed and identified as the diagnosis in a 55 year old man presenting with lymphopenia. However, it is dismayingthatwhen discussing investigations, Brass and colleagues state that an HIV test should be considered in patients with "high risk activities orchronic severe unexplained lymphopenia."UK national guidelines for HIV testing state that an HIV test should be carried out in the case of any unexplained blood dyscrasia, not just if chronic and severe. HIV seroconversion can cause reversible lymphopenia and the diagnosis can be missed if an HIV test is not taken initially.
