A recent analysis by Bartels and colleagues (1) among Medicare patients with rheumatoid arthritis (RA) revealed that although these patients are at increased risk of cardiovascular disease (CVD), primary lipid screening was performed in only 45%. Various explanations exist, but an interesting possibility is the complex relationship between lipids and RA, which may perplex physicians and lead them to doubt the significance of lipids in the pathogenesis of CVD in patients with RA. This complexity was highlighted in a Mayo Clinic study by Myasoedova et al (2), in which RA patients with lower total and low-density lipoprotein (LDL) cholesterol levels had a higher CVD risk than those with higher cholesterol levels.
