Cardiopulmonary bypass (CPB) has been at the cornerstone for surgical management of various heart disorders not amenable to medical therapy. The advent of a CPB machine way back in 1951 has revolutionized the practice of heart surgery (1). While there has been a significant improvement in patient outcomes after surgical procedures employing CPB, controversy still exists over the right mode of perfu-sion that should be utilized during surgery (2).A recent study by Gu et al. (3) randomized 32 patients undergoing CPB surgery to receive either pulsatile or nonpulsatile flow using the same centrifugal pump (Rotaflow, Maquet Cardiopulmonary, Hir-rlingen, Germany). The centrifugal pumps, by design, are engineered to provide nonpulsatile flow; however, significant pulsatility has been achieved in this study by Rotaflow pump as demonstrated by an average pulse pressure of 20-27 mm Hg.
