A 70-year-old man was admitted to the hospital due to high fever, along with pain and swelling in his right arm. His blood analysis results were: white blood cell (WBC) count, 19-58 x 10~9/l; calcium, 2-25 mmol/1; total protein, 72 g/1; albumin, 35 g/1; creatinine, 76-11 umol/1; beta_2-microglobulin, 5-8 mg/1 (normal, 0-68-1-65 mg/1); and C-reactive protein (CRP), 14-2 mg/1 (normal, <2-5 mg/1). No paraprotein was detected on serum protein electrophoresis and immunofix-ation. Serum concentration of granulocyte colony-stimulating factor (G-CSF) was 126 ng/1 (normal range, <39-0 ng/1). None of the tests revealed an infectious source. Magnetic resonance imaging (MRI) (Fig 1) of the right humerus showed a pathological fracture. Biopsy of the humerus showed abnormal plasma cells (Fig 2A).
